We may want to collect personal information from you, including information about your name, contact information, demographic, and medical history in order to provide necessary care.
Besides our staff, we share this information with WellSouth PHO and your own healthcare provider (General practitioner) in order to remain transparent in the delivery of your care.
We keep your information safe by storing it in Patient Medical System and only allowing access to clinical and admin staff who are essential to the care we provide. All Clinical Staff are legally bound by the Pacific Trust Otago Confidentiality Agreement.
You have the right to ask for a copy of any personal and health information we hold about you, and to ask for it to be corrected if you think it is wrong. What is HealthOne?
HealthOne is a South Island based secure electronic record that allows registered healthcare providers directly involved in your healthcare, to quickly access information such as your test results, allergies, medications, GP summaries and hospital information. HealthOne strictly adheres to the principles of the Privacy Act, 2020 as well as Rule 5 (Storage and Security of Health Information) set out in the Health Information Privacy Code 2020. Access is only possible via an approved highly secure healthcare information network which is regularly audited and tested.
